2. Patient’s Responsibilities:
   
   1. Provide information
   2. Follow treatment plan recommended by the Attending Physician
   3. Report unexpected changes on his condition
   4. Ask questions regarding his/her conditions
   5. Inform the Attending Physician for refusal of treatment and notify immediately
   6. Financial obligations to hospital and Doctors
   7. Follow Hospital Rules & Regulations
   8. To be considerate to the needs of other patient
